IVF Cost in Ahmedabad: What the Bill Is Actually Made Of

Two clinics can quote figures a lakh apart for what sounds like the same treatment. Here is what an IVF bill is made of, which parts vary with your own biology, and what usually sits outside the headline number.

Two clinics in the same city can quote figures that differ by more than a lakh for what sounds like the same treatment. Usually neither is lying. They are quoting different things.

This page sets out what an IVF bill is actually made of, which parts are fixed and which vary with your own biology, and what tends to sit outside the headline number. It starts with what IVF costs here.

What IVF costs at Cosmic

IVF at Cosmic Women’s Hospital, Naroda Cost
One IVF cycle From Rs 1,20,000
Included in the cycle figure Not included, billed separately
  • Consultations through the cycle
  • Stimulation injections
  • Monitoring scans (sonography)
  • Egg collection (ovum pick-up), with the anaesthesia for it
  • The embryology laboratory work
  • Frozen embryo transfer (FET)
  • Embryo freezing, with storage for 12 months
  • Blood tests (the reports)
  • Tablets

Blood tests and tablets sit outside the figure, which is why they are named here rather than left for you to discover on the bill. The final figure for your cycle depends on the protocol and the dose your treatment needs, which is why Rs 1,20,000 is a starting figure and not a fixed one. Your own figure, and anything your treatment needs beyond the standard cycle, is set out for you in writing before treatment starts. The items that most often sit outside a headline IVF figure are listed further down this page, so you know what to ask about, wherever you are treated.

The single most useful question to ask

Not “how much is IVF”, but: “what is included in that figure, and what will be billed separately?”

An IVF cycle is not one product. It is a package of consultations, drugs, scans, an operating-theatre procedure, laboratory work, and often storage. A quote covering four of those six will always look cheaper than a quote covering all six.

What the bill is made of

Component What it covers What makes it vary
Pre-cycle workup Hormone profile, AMH, thyroid and prolactin, semen analysis, screening bloods for both partners, assessment of the uterine cavity. Largely fixed. Extra tests if there is a history of failed cycles or pregnancy loss.
Stimulation drugs Gonadotrophins to grow multiple follicles, plus the drugs that prevent premature ovulation and the trigger injection. The largest single variable. Depends on your ovarian reserve, your age and your protocol. A woman needing a high dose for twelve days will spend far more than one needing a low dose for nine.
Monitoring Serial ultrasound scans and blood tests through the stimulation, typically four to six visits. Number of scans varies with how your ovaries respond.
Egg retrieval Theatre, anaesthesia, the procedure itself. Largely fixed.
Laboratory: fertilisation Conventional IVF, or ICSI where a single sperm is injected into each egg. ICSI costs more than conventional IVF and is used for male factor infertility or previous fertilisation failure — not routinely.
Laboratory: embryo culture Growing the embryos to day 3 or to blastocyst on day 5. Blastocyst culture may add cost; it is chosen on embryo numbers and quality, not on preference.
Embryo transfer The transfer procedure and the luteal support that follows. Fixed for a fresh transfer. A frozen transfer in a later cycle is a separate charge.
Freezing and storage Vitrification of surplus embryos, then an annual storage fee. Optional in principle, but if you have good surplus embryos, freezing them is what makes a second attempt affordable.

The parts that are often outside the headline figure

  • A frozen embryo transfer. At many clinics the transfer of frozen embryos is a fresh set of charges: drugs, monitoring, thawing and the transfer itself. At Cosmic, the frozen embryo transfer is part of the cycle figure, so ask any clinic you are comparing whether theirs is.
  • Embryo storage. Freezing is often billed separately, and storage is usually charged by the year. At Cosmic, freezing and the first 12 months of storage are included in the cycle figure; storage after that is renewed separately.
  • Surgery found on the workup. A hysteroscopic polypectomy, a septum resection, removal of a submucous fibroid or a laparoscopic procedure for hydrosalpinx. These are done before the cycle because they change the outcome, and they are billed separately.
  • Surgical sperm retrieval. Where there is azoospermia, retrieval by PESA, TESA or micro-TESE is a separate procedure.
  • Genetic testing of embryos. PGT-A is a substantial additional cost and is not indicated for most couples.
  • Donor gametes. Where donor eggs or sperm are used, ART Act requirements and bank charges apply and are quoted separately.
  • Treating a complication. Ovarian hyperstimulation syndrome is uncommon with modern protocols but, if it happens, may need admission.

Why “package deals” need reading carefully. A fixed all-inclusive package can be genuinely reassuring, because it caps your exposure. It can also be structured so that the commonest additional costs — the drugs, the ICSI, the freezing — sit outside the package. Ask for the exclusion list in writing, not the inclusion list. The exclusions are where the money is.

Why quotes differ so much between clinics

Drug protocol and drug brand

Recombinant and urinary-derived gonadotrophins differ substantially in price and are not identical products. Which is appropriate depends on your response, not on cost alone, but you are entitled to know which is being used and why.

Whether ICSI is default or selective

Some centres use ICSI for nearly every case. It is the right choice for male factor infertility and after previous fertilisation failure. Used routinely where the semen parameters are normal, it adds cost without adding benefit for most couples.

Blastocyst culture and freeze-all policies

A freeze-all strategy — freezing every embryo and transferring in a later cycle — is medically correct in specific situations, such as a high risk of hyperstimulation or a raised progesterone at trigger. Applied as a blanket policy, it moves cost from cycle one to cycle two.

What is counted as a “cycle”

Some quotes describe one egg retrieval. Others describe a retrieval plus one frozen transfer. Comparing them directly is comparing different products.

Questions to put to any clinic before you pay

  • Is this quote for one egg retrieval, or does it include a subsequent frozen transfer?
  • Are the stimulation drugs included, or estimated separately? What is the estimated range for someone with my AMH?
  • Is ICSI included, and is it indicated in my case or being used by default?
  • What will freezing and one year of storage cost?
  • What happens to the fee if the cycle is cancelled before retrieval, or if no eggs are obtained?
  • Will I be given a written, itemised estimate, and will I be told before any additional charge is incurred?

The last one matters most. A clinic that will put its estimate in writing before you start is a clinic that has thought about the answer.

At Cosmic. Every couple is given a written, itemised estimate after the workup is complete and before the cycle begins, with the drug component shown as a range because it depends on how your ovaries respond. Additional charges are discussed with you before they are incurred, not added to the final bill. We will also tell you plainly when we think IVF is not the treatment you need — the cheapest IVF cycle is the one you did not have to do.

A note on paying for it

Most fertility treatment in India is self-funded; standard health insurance policies commonly exclude it, though some corporate group policies and newer products now include limited cover. It is worth reading your own policy wording rather than assuming either way, and asking your employer’s HR whether the group policy has a fertility benefit.

Where a couple is deciding between one well-investigated cycle and two rushed ones, the evidence is on the side of the first. Money spent on finding and correcting a treatable problem before the cycle is not money taken away from the treatment. It is usually what makes the treatment work.
